Terms + Conditions / Release from Liability

I wish to register to participate in one or more classes or programs offered by the Metropolitan Center for the Visual Arts (VisArts) or to register my child who is under the age of 18. I understand that some of the activities involve risks including, but not limited to, bodily injury, death or property damage. I also understand the importance of following all instructions, heeding all safety warnings and not engaging in any activity I am not able to perform safely based on my physical condition or otherwise. I hereby consent to participate in the program that I have registered for and assume those risks on behalf of myself or for the child identified in this registration.

I certify that I understand the terms of this Release from Liability, that I am age eighteen (18) years or older or the parent/legal guardian of the student identified in the registration form and that I am authorized to execute this release from liability on his/her behalf.

This release is intended to discharge in advance, VisArts, its officers, officials, employees, volunteers or agents from liability, even though that liability may arise out of perceived negligence on the part of persons mentioned above. It is further understood and agreed that this waiver, release and assumption of risk is to be binding on my heirs and assignees.

I hereby give my consent to have the applicant treated by emergency medical personnel, a physician or surgeon in case of sudden illness or injury while participating in the registered for activity. It is understood that VisArts will provide no medical insurance for such treatment and that the cost thereof will be at my expense.

I understand VisArts photographs and videotapes students for possible use in its publications, applications for funding, website, and other development/public relations purposes. I acknowledge that no compensation is provided to individuals who appear in photographs or videotapes. In an effort to protect my (or my child’s) privacy, I understand that names will not be used with any publication. I understand that VisArts will ask my permission before taking my photo.

I understand and acknowledge that by registering or RSVPing for a class, workshop, program, or event, I am consenting to being added to VisArts’ email list. I understand that VisArts will email essential updates regarding my registration and other information about VisArts, and that VisArts respects my privacy and will never sell or share my email with third parties.
